In addition, please download our "Bank Authorization Form". Please complete and sign, and fax it to 800-952-8281.
You may also wish to contact your Bank, advising that we will be submitting this authorization from you as part of our credit check and the opening of a line of credit with us.
A copy of your State's Exemption Certificate should be included and faxed with the credit information.
ALLOW 10 BUSINESS DAYS TO REVIEW AND ESTABLISH AN ACCOUNT.
All information will be kept CONFIDENTIAL.
